GUIDE Partner for Alzheimer’s and Dementia Care

At Assisting Hands Home Care, we understand the emotional and financial toll of caring for loved ones with Alzheimer’s disease and other forms of dementia. What is GUIDE?

The GUIDE Model (short for Guiding an Improved Dementia Experience) is a new nationwide Medicare-linked program in the United States designed to improve the quality of care and support for people living with dementia and their unpaid caregivers. It’s run by the Centers for Medicare & Medicaid Services (CMS) through the CMS Innovation Center as a voluntary, multi-year model test.

What the GUIDE Model is:

  • A comprehensive, coordinated dementia care model focused on improving health and quality of life for people with dementia and reducing strain on caregivers.
  • It began on July 1, 2024, and is planned to run for eight years as a nationwide voluntary pilot.
  • It tests an alternative payment model (APM), moving beyond traditional fee-for-service Medicare toward monthly payments that support a broader array of care and support services.

Key Features of the GUIDE Model

  1. Alzheimer's Dementia GUIDE Program Partner Ellis, Tarrant TX Comprehensive Care for People with Dementia
  • Person-centered assessments and individualized care plans.
  • Coordination across clinical and non-clinical services (e.g., meals, transportation).
  • Ongoing monitoring and support with 24/7 access to a support line.
  • Navigation help via a Care Navigator who connects patients and families to needed services.
  1. Support for Unpaid Caregivers
  • Caregiver education and training on best practices for dementia care.
  • Emotional and practical support resources.
  • Respite services (temporary breaks for caregivers), with Medicare paying up to a set amount yearly.
  1. Health-Related Social Needs Screening
  • Participants are screened for psychosocial and social needs and connected to community resources.

Who It Helps

  • Medicare fee-for-service beneficiaries diagnosed with dementia (including Alzheimer’s and related conditions).
  • People living at home or in the community (not in long-term nursing facilities, hospice, or certain managed plans).
  • Their unpaid caregivers, often family or friends, who take on substantial physical, emotional, and logistical care roles.

How It Works

  • Participating health care organizations establish dementia care programs that meet GUIDE’s care delivery requirements.
  • Medicare pays these organizations on a monthly basis to deliver coordinated services beyond traditional billing codes.
  • The model includes two tracks: one for established programs and one for new programs that are building capacity.

Goals of the GUIDE Model

  • Help people with dementia stay safely in their homes longer.
  • Improve quality of life and health outcomes for patients and caregivers.
  • Reduce unnecessary hospitalizations and costly nursing home placements.
  • Test a better payment method that could shape future Medicare dementia care policy.

Eligibility Criteria

To receive dementia care services under the GUIDE Model, a person must meet all of the following:

  1. Have a Dementia Diagnosis
    • Dementia must be confirmed by a clinician who is part of a participating GUIDE dementia care program.
  2. Be Enrolled in Traditional Medicare
    • Must be enrolled in Medicare Parts A and B (Original Medicare) with Medicare as the primary payer.
    • Medicare Advantage plans (including Special Needs Plans), PACE (Program of All-Inclusive Care for the Elderly), and Medicare Hospice enrollment disqualify someone from receiving GUIDE services.
  3. Must Live at Home or in a Community
    • The patient must live at home or in an independent/ assisted living community to be eligible. Beneficiaries living in long-term care facilities or nursing homes are not eligible.
  4. Voluntary Alignment to a GUIDE Participant
    • The person must choose to align with a participating dementia care program in their community. CMS verifies eligibility after being referred and consented.

How the GUIDE Model Helps Family Caregivers

The GUIDE Program is designed to ease three of the most challenging pressures faced by family caregivers:

  1. Overwhelming Care Demands
    Providing ongoing care places a heavy load on caregivers physically, emotionally, mentally, and financially. From assisting with daily personal needs and managing household responsibilities to coordinating medical appointments and handling expenses, caregivers often find themselves stretched across every aspect of a loved one’s life.
  2. Chronic Stress and Emotional Strain
    Many caregivers experience persistent stress, burnout, and feelings of isolation. Over time, this emotional toll can erode their own health, increasing the risk of exhaustion, illness, and the need for medical care themselves.
  3. Limited Guidance and Fragmented Support
    Caregivers are frequently left to navigate complex care systems on their own. Without consistent coordination, training, or easy access to community resources, they may struggle to manage the growing and changing needs of the person in their care.

1. Coordinated, Person-Centered Dementia Care

Participants receive ongoing, comprehensive assessment and care planning tailored to their needs. An interdisciplinary care team helps guide care and monitor progress over time instead of fragmented, one-off care.

2. Dedicated Care Navigation

Each person with dementia and their caregiver are paired with a care navigator who:

  • Helps coordinate medical and non-medical services
  • Connects families to local community resources (like meals, transportation, etc.)
  • Provides proactive outreach and support, so families aren’t left to manage everything alone.

3. 24/7 Access to Support

Aligned beneficiaries and caregivers have 24/7 access to a support line or care team member to ask questions, manage issues, and get help when they need it, day or night.

4. Caregiver Education and Support

Caregivers receive training, tools, and education on best practices in dementia care, helping them feel more confident and prepared in their role.

5. Respite Services

The model includes funds to help pay for respite care services that offer temporary relief for unpaid caregivers so they can rest, run errands, or attend to personal needs. For eligible beneficiaries this can be up to about $2,500 per year (exact amount may vary by implementation).

6. Support for Daily Living and Transitions

GUIDE care teams assist with medication management, transitional care, and screening for health-related social needs (like food insecurity or transportation), helping reduce gaps in care and ensure people get support across all areas of life.

7. Helps People Stay at Home Longer

By giving families support and access to coordinated care, the model aims to prevent or delay unnecessary hospitalizations and long-term nursing home placement, allowing people with dementia to stay safely in their homes and communities.

8. No Additional Cost to Eligible Beneficiaries

Services provided through the GUIDE Model are fully covered by Medicare for eligible participants; aligned beneficiaries don’t face separate charges for these enhanced dementia care supports.
