Medicare's Free GUIDE Program for Dementia Care

Assisting Hands is excited to introduce a free groundbreaking Medicare dementia care program called GUIDE. This initiative provides 72+ hours of no-cost respite care, virtual nurse support, and caregiver assistance to families within our community caring for individuals with dementia.

This marks the first time Medicare has launched such a program on a nationwide scale, and Assisting Hands takes pride in supporting caregivers in finding their own respite care.

We would love to share how families are benefiting from this program and how yours may too.

Who Is Eligible

  • Has moderate to severe dementia (diagnosis can be provided)
  • Enrolled in Medicare Parts A & B (not Advantage)
  • Lives at home or in independent/assisted living
  • Not currently on hospice or a skilled nursing resident (short-term stay is OK)

What You Get

  • 72 hours of no-cost respite care for the caregiver
  • Your own dementia-trained nurse with video/phone check-ins
  • 24/7 virtual access to your care team
  • Family education and 1:1 caregiver coaching
  • Medication review
  • Care coordination with doctors, specialists & equipment providers
  • Management of chronic conditions (heart disease, diabetes, cancer, etc.)
Good to know: VA patients, dually eligible Medicaid patients, memory care, and palliative care patients may also qualify.
