A Night at the Rink, A Stronger Partnership for Patients
At Assisting Hands Home Care, relationships matter—especially the ones that help seniors and families feel supported during major life transitions. Recently, Tim and I (Lisa) had the chance to attend a hockey game with Sean from Northern Utah Rehabilitation Hospital, and it turned into one of those community moments that reminds us why collaboration is so important.
Yes, we cheered, laughed, and enjoyed a great night out. But beyond the fun, the real win was connecting in a way that strengthens the bridge between rehab care and safe recovery at home—for the people we all serve across Davis and Weber Counties.
Why Hospital-to-Home Transitions Matter So Much
A rehab stay can be a powerful step forward—whether someone is recovering after a fall, a surgery, a stroke, illness, or simply rebuilding strength after a difficult season. But what happens next is just as important.
Many families tell us the same thing:
-
“They’re coming home, but they’re not back to normal yet.”
-
“We’re worried about falls.”
-
“We don’t want to hover… but we can’t leave them alone.”
-
“They’re supposed to do exercises, eat well, and stay on schedule—but it’s overwhelming.”
That’s where continuity of care makes a real difference. When rehab teams and in-home care providers communicate well and understand each other’s roles, the transition home can feel far less stressful—and a lot safer.
What a Strong Rehab + Home Care Partnership Looks Like
Our goal is simple: help people stay independent at home while reducing risk and easing the load on family caregivers.
When someone discharges from rehab, in-home support can help with:
-
Safe mobility support (walking assistance, transfers, fall prevention routines)
-
Personal care (bathing, grooming, dressing, toileting)
-
Medication reminders and daily routine support
-
Meal prep and hydration encouragement
-
Light housekeeping to keep pathways clear and reduce hazards
-
Transportation to follow-up appointments, when appropriate
-
Companionship and encouragement (recovery can be emotional, not just physical)
We’re not there to replace therapy—we’re there to support the day-to-day consistency that helps therapy “stick” at home.
The Power of Community Connections
Getting to spend time with Sean outside of a formal setting was meaningful. It gave us space to talk about shared goals, the realities families face after discharge, and the kind of support that helps patients feel confident again once they’re back in their own environment.
We’re grateful for healthcare partners like Northern Utah Rehabilitation Hospital who care deeply about patient outcomes—and who understand that what happens at home is a critical part of recovery.
