Respite is for families caring at home for someone with moderate or severe dementia on Original Medicare. We are an approved respite provider for PocketRN&#8217;s GUIDE program, and we help eligible families with the paperwork.<\/p>\n<p>So if you heard that Medicare may pay for a break, it can be true, with limits. CMS, the federal agency that runs Medicare, says dementia caregivers commonly report high stress and depression. GUIDE was built in part to support them.<\/p>\n<h2>What is the GUIDE program?<\/h2>\n<p>GUIDE stands for Guiding an Improved Dementia Experience. CMS started it on July 1, 2024, and set it to run for eight years, through June 30, 2032. Joining is voluntary. Your loved one keeps regular Medicare benefits and can still see any doctor or hospital that accepts Medicare.<\/p>\n<p>Each GUIDE program puts a dementia care team around the person and the family. CMS requires programs to offer:<\/p>\n<ul>\n<li>A care team member who coordinates care and links you to community help such as meals and rides.<\/li>\n<li>A support line you can call at any hour, every day of the week.<\/li>\n<li>Caregiver training, support groups and one-on-one support calls.<\/li>\n<li>Respite care for families who qualify.<\/li>\n<\/ul>\n<p>Much of this can happen by phone or video. For respite, all of these must be true:<\/p>\n<ul>\n<li>A clinician on the GUIDE program&#8217;s team confirms the dementia.<\/li>\n<li>Original Medicare (Part A and Part B) is the main coverage. Medicare Advantage, Special Needs Plans and PACE do not qualify.<\/li>\n<li>Your loved one is not getting Medicare hospice care.<\/li>\n<li>They live in a private home, not long term in a nursing home and not in a memory care unit.<\/li>\n<li>They are not signed up with a different GUIDE program.<\/li>\n<\/ul>\n<p>The dementia respite benefit has two more conditions. The dementia must be moderate or severe, which the program measures during its assessment. A relative or other unpaid caregiver must also be helping with daily tasks, from meals to medicines to getting dressed.<\/p>\n<p>Signing up is not automatic. The program explains GUIDE, asks for your consent, does an assessment and sends your loved one&#8217;s information to Medicare. Medicare confirms eligibility, and the program tells you when your loved one is enrolled.<\/p>\n<h2>How does the yearly respite budget work?<\/h2>\n<p>GUIDE pays for dementia respite as a dollar amount each year, not a set number of hours. CMS adjusts the limit for inflation each year. That is why some pages, including part of CMS&#8217;s own site, still show the original $2,500. The current figure is in <a href=\"https:\/\/www.cms.gov\/priorities\/innovation\/files\/guide-payment-methodology-paper.pdf\" target=\"_blank\" rel=\"noopener\">CMS&#8217;s 2026 GUIDE payment paper<\/a>.<\/p>\n<h2>What is our part in GUIDE?<\/h2>\n<p>GUIDE dementia respite has to come from the program itself or from a partner CMS has approved. Families cannot spend it with just any home care agency. We are an approved respite provider for <a href=\"https:\/\/pocketrn.com\/guide-program\" target=\"_blank\" rel=\"noopener\">PocketRN&#8217;s GUIDE program<\/a>. PocketRN appears on CMS&#8217;s list of GUIDE programs, serves Texas, and works with families by phone and video.<\/p>\n<p>If your loved one has dementia and Original Medicare, we point you to PocketRN and help with the paperwork. PocketRN does the assessment and Medicare confirms eligibility, so we cannot promise approval, hours or a dollar amount.<\/p>\n<p>You do not have to put off help while the forms move along. Families can start private-pay care with us while GUIDE or VA benefits are pending. Care can usually start within a day or two, and often the same day. Our caregivers are non-medical. Depending on the plan, a visit can include:<\/p>\n<ul>\n<li>Company and supervision, so your loved one is not alone while you are out.<\/li>\n<li>Meals and snacks served at the usual times.<\/li>\n<li>Help with bathing, dressing or toileting.<\/li>\n<li>Medication reminders (reminders only).<\/li>\n<li>A walk, music, an old hobby or a photo album.<\/li>\n<li>Light housekeeping, such as dishes or laundry.<\/li>\n<\/ul>\n<p>The National Institute on Aging suggests keeping meals and baths on a regular schedule for people with Alzheimer&#8217;s. Bring that routine to the free in-home consultation, along with what calms your loved one. You can read more about <a href=\"https:\/\/assistinghands.com\/135\/texas\/nesanantonio\/services\/alzheimers-and-dementia-care\/\">everyday dementia support from our caregivers<\/a>.<\/p>\n<p>Our caregivers do not provide medical care, such as wound care, injections or giving medicines. While they are there, you might sleep, keep your own doctor&#8217;s appointment, or have lunch with a friend. That is the point of respite.<\/p>\n<h2>What if the respite money runs out?<\/h2>\n<p>Once the yearly limit is used, GUIDE stops paying for respite until the next July 1. The program does not have to keep offering respite after that. You can pay privately for more. CMS does not allow the program to require it or to bill you for the difference.<\/p>\n<p>Medicare does not pay for long-term care, which includes ongoing personal care like bathing or dressing. To keep dementia respite going after the GUIDE money is gone, families can look at these:<\/p>\n<ul>\n<li><strong>Private pay.<\/strong> Many older adults use savings, a pension or other retirement income. Our <a href=\"https:\/\/assistinghands.com\/135\/texas\/nesanantonio\/services\/respite-care-services\/\">private-pay respite visits<\/a> can pick up where GUIDE leaves off.<\/li>\n<li><strong>Long-term care insurance.<\/strong> These policies can cover help with daily activities at home, and some cover respite. Policies differ on who may provide the care, so ask the insurer what yours pays for.<\/li>\n<li><strong>VA Aid and Attendance.<\/strong> This monthly payment is added to a VA pension for qualified veterans and survivors who need help with daily activities like bathing or dressing. We can walk you through the application and help get it started. VA approvals can take months longer than VA&#8217;s published averages, so plan for a wait.<\/p>\n<h2>How do you get started?<\/h2>\n<ol>\n<li>Call us at (210) 405-4059 for a free in-home consultation. We will talk through your loved one&#8217;s needs and whether GUIDE may fit.<\/li>\n<li>If it may, we point you to PocketRN&#8217;s GUIDE program and help with the paperwork.<\/li>\n<li>PocketRN starts with a virtual assessment. It looks at the stage of the dementia and how much strain the caregiver is under.<\/li>\n<li>PocketRN sends your loved one&#8217;s information to Medicare, which confirms eligibility. PocketRN lets you know when your loved one is enrolled.<\/li>\n<li>While you wait, private-pay care can begin.<\/li>\n<\/ol>\n<p>Have your loved one&#8217;s Medicare card ready, plus a list of doctors and medicines. Any records that mention dementia help too, since CMS&#8217;s fact sheet for families mentions a referral from a doctor.<\/p>\n<h2>Common questions<\/h2>\n<h3>Do we need a dementia diagnosis before we call?<\/h3>\n<p>No. A clinician on the GUIDE program&#8217;s team has to confirm the dementia, and PocketRN says it can help families get a diagnosis when appropriate.<\/p>\n<h3>Can someone on Medicare Advantage use GUIDE?<\/h3>\n<p>No. Only people on Original Medicare can join, and enrolling in a Medicare Advantage plan ends GUIDE. A person can leave Medicare Advantage for Original Medicare during Open Enrollment (October 15 to December 7) or from January 1 to March 31. Call Texas&#8217;s free Medicare helpline before switching.<\/p>\n<h3>What if my loved one lives in assisted living?<\/h3>\n<p>They can join a GUIDE program only if the community has a CMS-approved partnership with it. Even then, CMS does not give assisted living residents GUIDE dementia respite, and PocketRN serves people who live at home.<\/p>\n<h3>We already joined another GUIDE program. Can we change?<\/h3>\n<p>Yes. A person can be enrolled with only one GUIDE program at a time, but switching is allowed.
