Our trained Certified Care Coordinator coordinates all aspects
of your patient’s recovery, coordinating care with all of your
patient’s health providers. When you discharge your patient,
then won’t be on their own… Our Certified Care Coordinators
are trained to improve the healing process and minimize pain
and discomfort by:

  • Ensuring compliance with discharge care instructions as well as oversight of medication regimen and care plan requirements
  • Ensuring your patients have transport and accompaniment to vital doctor’s and other health care provider’s appointments
  • Providing respite and support to friends and family
  • Providing the support and social engagement necessary to increase well being

Our caregivers can assist you with:

  • Hygiene, ambulation, and transfers
  • Changing Ice Bags
  • Shopping, meal preparation, and eating assistance
  • Light housekeeping and routine errands
  • Transport to and from medical, therapy, or other critical appointments
  • Medication reminders
  • Help with range-of-motion exercises, and other treatment modalities
  • Patient assessment and monitoring of vital signs
  • Assessing for fall risk due to unsteady gait or dizziness and implementing a prevention plan

Personalized Approach

  • We are available 24 hours a day/365 days a year.
  • Services are provided at the hospital, independent or
    assisted living community, skilled nursing facility,
    or at home.
  • We provide flexible schedule with hourly, day or night
    care, as well as around the clock service.
  • The Plan of Care will be based on your doctor’s
    instructions and your specific needs and supervised
    by our Certified Care Coordinator.
