To participate, individuals must:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Have a formal diagnosis of dementia (including Alzheimer\u2019s disease or other forms).<\/li>\n\n\n\n<li>Be enrolled in Medicare Parts A and B.<\/li>\n\n\n\n<li>Live in the community (not a long-term nursing home).<\/li>\n\n\n\n<li>Have an identified caregiver or support person, when possible, to participate in the care plan.<\/li>\n<\/ul>\n\n\n\n<p>CMS intends for GUIDE to reach patients across all stages of dementia, providing the right level of support at the right time.
