Assisting Hands West Orange will provide a caregiver up to 24 hours a day and as long needed to ensure safety and optimal care for someone who comes home post-surgical or post-illness.
\n\u2022 Caregivers can transition with clients while in the hospital, rehabilitation or nursing facility and on to home.
\n\u2022 Caregivers help ensure compliance with discharge instructions.
\n\u2022 Caregivers assist with home safety by helping with everyday tasks as you or a loved one adjusts to being back at home.<\/p>\n

Transitional Care<\/h2>\n

We utilize principles from nationally successful readmission prevention programs to keep you or your loved one from a returning to the hospital after surgery or illness. Inadequate transition care, non-compliance with discharge instructions and medication errors\/non-compliance are all top reasons for readmissions.<\/p>\n

The penalties are based on readmissions of Medicare patients who initially came to the hospital with specific diagnoses that include:
