Continuing with the theme this month of families connecting for the holidays, I want to share this 5-step tool for creating a care plan for your loved one.
I often see families struggling with getting on the same page together. We always recommend a care meeting where everyone can share their concerns and ideas. This is often the first time some family members have heard each other’s concerns! I am happy to act as a moderator in these meetings – sometimes a third-party professional can help guide the discussion and provide resources that families didn’t know existed.
Two areas that I focus on are goals and budget. Goals of care are often nebulous until the family, including the proposed client, can air their concerns and really articulate what is most important to them. The process of defining goals and writing them down can lead to clarity and unity of purpose. It also helps stretch your care dollars to fit your budget.
