UTIs in Seniors: Symptoms, Risks & When to Get Help

Urinary tract infections (UTIs) are common, but in older adults, they can be surprisingly easy to overlook. A senior may not always complain of the familiar burning or pain associated with a UTI. Instead, families may notice a sudden change in behavior, increased confusion, weakness, or changes in daily routines.

For families caring for an aging parent, recognizing these changes is very important.

What Is a UTI?

A urinary tract infection occurs when bacteria enter the urinary system and cause an infection. UTIs can affect the bladder, urethra, ureters, or kidneys.

Common symptoms can include:
• Burning or pain when urinating
• A frequent or urgent need to urinate
• Passing small amounts of urine
• Pelvic or lower abdominal discomfort
• Blood in the urine
• Urine that appears unusual

If an infection reaches the kidneys, symptoms can become more serious, including fever, chills, back or side pain, nausea, and vomiting.

Why UTIs Can Be Different in Older Adults

One reason UTIs can be missed in seniors is that symptoms may not always be obvious.

A family member might instead notice:
• Sudden confusion or changes in attention
• Unusual sleepiness or agitation
• A sudden change in behavior
• Increased difficulty managing normal routines
• New weakness or difficulty getting around

A UTI can be associated with delirium, which is a sudden change in thinking and attention. However, confusion should not automatically be assumed to be caused by a UTI. Delirium can have many causes, including other infections, medication effects, dehydration, and other medical conditions.

A sudden change in mental status deserves medical attention.

If your loved one develops symptoms that could indicate a UTI, contact their healthcare provider for evaluation. A healthcare professional can determine whether a UTI is actually present and what treatment is appropriate.

Seek prompt medical attention if symptoms include:
• High fever
• Chills or shaking
• Back or side pain
• Nausea or vomiting
• Blood in the urine
• Sudden or significant confusion
• A rapid change in your loved one’s condition

These symptoms can indicate a more serious infection or another medical problem requiring prompt evaluation.

What Can Families Do at Home?

Families can help by paying attention to what’s normal for their loved one. Simple steps include:

Keep Fluids Available
Encourage appropriate fluid intake throughout the day. Hydration is especially important for older adults, although people with certain medical conditions may have fluid restrictions. Families should follow their healthcare provider’s recommendations.

Watch for Changes
Don’t dismiss sudden changes in behavior, mobility, appetite, or bathroom habits as “just getting older.”

Support Bathroom Routines
A senior with limited mobility may delay using the bathroom because getting there is difficult. Making the bathroom accessible and providing appropriate assistance can help.

Keep Track of Symptoms
If something changes, write down when it started and what you’ve noticed. This information can be useful when speaking with a healthcare provider.
