What is the GUIDE Program?

The GUIDE program is not a new insurance policy or a specific medical procedure. Instead, it is an integrated model of care management. When an eligible individual enrolls with an approved, Certified GUIDE Provider like Assisting Hands Home Care, they unlock a supportive infrastructure designed to coordinate their health, safety, and daily care requirements. Core benefits of the program include:

  • A Dedicated Care Navigator: A professional who acts as your personal advocate, helping you coordinate doctors, organize prescriptions, and access local community resources.

  • 24/7 Crisis Support: Access to a helpline staffed by clinical professionals who can assist with sudden behavior changes or medical concerns, keeping patients out of unnecessary emergency room visits.

  • Caregiver Training and Education: Practical strategies to help family members handle the changing behavioral and cognitive symptoms of dementia.

  • Personalized Care Plans: A comprehensive assessment of the patient’s home safety, physical condition, and cognitive baseline to build a customized roadmap for the family.

The Respite Care Benefit: 72 Hours of Annual Relief

Perhaps the most profound element of the GUIDE program is its recognition of caregiver burnout. Caregiving is a full-time job that carries intense emotional and physical strain. To alleviate this, the GUIDE model provides an annual allocation of up to 72 hours of fully covered respite care services.

This benefit allows family caregivers to take a necessary break, knowing their loved one is in safe, professional hands. Because Assisting Hands Home Care is an approved, Certified Provider, these hours are fully covered by Medicare at no cost to your family. Who is Eligible for the GUIDE Program?

Because this is a specialized program run by the Innovation Center, CMS has established strict criteria to ensure the resources reach those who need them most. To qualify for the GUIDE program, the patient must meet the following checklist:

  • Confirmed Diagnosis: Must have a formal clinical diagnosis (or suspected condition) of dementia or Alzheimer’s disease.

  • Insurance Track: Must be enrolled in Traditional Medicare (Part A and Part B) as their primary coverage.

  • Exclusions: Patients are currently ineligible if they are enrolled in a Medicare Advantage plan, a PACE program, or are receiving active hospice care.

  • Living Situation: Must live in a community setting (such as a private home or assisted living). Long-term nursing home residents do not qualify.
