Medicare's Free GUIDE Program for Dementia Care

Assisting Hands is excited to collaborate with Pocket RN to introduce a free groundbreaking Medicare dementia care program called GUIDE. This initiative provides 72 hours of no-cost respite care, virtual nurse support, and caregiver assistance to families within our community caring for individuals with dementia.

This marks the first time Medicare has launched such a program on a nationwide scale, and Assisting Hands takes pride in supporting caregivers in finding their own respite care.

Who Is Eligible

  • Has moderate to severe dementia (diagnosis can be provided)
  • Enrolled in Medicare Parts A & B (not Advantage)
  • Lives at home or in independent/assisted living
  • Not currently on hospice or a skilled nursing resident (short-term stay is OK)

  • Your own dementia-trained nurse with video/phone check-ins
  • 24/7 virtual access to your care team
  • Family education and 1:1 caregiver coaching
  • Medication review
  • Care coordination with doctors, specialists & equipment providers
  • Management of chronic conditions (heart disease, diabetes, cancer, etc.)
Good to know: VA patients, dually eligible Medicaid patients, memory care, and palliative care patients may also qualify.
