Assisting Hands is a proud partner with the Department of Veterans Affairs as a provider in the Community Care Network (CCN).
The VA CCN provides supportive services to help chronically ill or disabled eligible Veterans of any age remain in their home. These services are referred to as home care and are considered extended care services prescribed by and under the direction of a VA physician.
Veterans may be eligible for professional home care from a VA-approved agency wherever they call home. This program is for Veterans who need personal care services and help with activities of daily living. This program is also for Veterans who are isolated, or their main caregiver is experiencing burden.
A Homemaker/Home Health Aide is a trained person who can come to a Veteran’s home and help the Veteran take care of themselves and their daily activities. Homemakers/Home Health Aides are not nurses, but they are supervised by a registered nurse who will help assess the Veteran’s daily living needs.
Services are based on your assessed needs. To qualify, Veterans must get a referral for services from their VA doctor at their local VA clinic.
