Every family is connected with their own dementia-trained provider/care manager who provides regular phone or video check-ins to monitor health and well-being.

24/7 Virtual Access to Care Support

Questions do not only come up during office hours. Dementia caregiving can feel unpredictable. That is why families have 24/7 virtual access to their care team whenever guidance is needed.

Generally, individuals may qualify if they:

  • Must be diagnosed with dementia
  • Must have active coverage under both Medicare Part A and Medicare Part B
  • Live at home or independently in a Retirement Community or in certain assisted living facility
  • Not under Hospice or enrolled in PACE program
