GUIDE, short for Guiding an Improved Dementia Experience, is a voluntary Medicare program run by the CMS Innovation Center that pays for coordinated dementia care, caregiver training, and respite services for eligible seniors living at home. Rather than leaving families to piece together fragmented appointments and community resources on their own, GUIDE funds a dedicated care team that manages the whole picture.
The program launched on July 1, 2024, and is scheduled to run for eight years as a nationwide test of a better way to deliver and pay for dementia care. Instead of the traditional fee-for-service approach, Medicare pays participating dementia care programs a monthly amount per patient, which covers services that standard Medicare billing never reached: care navigation, 24/7 support access, caregiver education, and funded respite breaks.

Who Qualifies for the GUIDE Program in Florida?
To qualify for GUIDE services, a senior must have a confirmed dementia diagnosis, be enrolled in Traditional Medicare (Parts A and B), and live at home or in a community setting rather than a nursing home. All of the following conditions apply:
- A dementia diagnosis confirmed by a GUIDE clinician: Alzheimer’s disease and related dementias all qualify; the diagnosis must be verified by a clinician within a participating GUIDE dementia care program.
- Traditional (Original) Medicare as the primary payer: Enrollment in a Medicare Advantage plan, a Special Needs Plan, PACE, or the Medicare hospice benefit makes a beneficiary ineligible for GUIDE.
- Living at home or in the community: Seniors residing in long-term nursing facilities are not eligible.
- Voluntary enrollment. The senior or their representative chooses to align with a participating GUIDE program, and CMS confirms eligibility after referral and consent.
