Medicare GUIDE Model & Dementia Care Support
Caring for someone living with dementia can be challenging for both families and unpaid caregivers. Fortunately, families may have access to programs designed to provide additional support and help them navigate dementia care.
One program families may encounter is the Medicare GUIDE Model.

What Is the Medicare GUIDE Model?
The Guiding an Improved Dementia Experience, or GUIDE, Model is a Medicare GUIDE Model designed to improve support for people living with dementia and their unpaid caregivers.
The model focuses on helping families navigate dementia care, coordinate services, and access resources.
Services are provided through participating organizations, and eligibility and availability may vary.
What Kind of Support May Be Available?
Depending on the participating organization and an individual’s circumstances, support may include:
- Care navigation
- Help understanding available resources
- Caregiver support
- Education
- Coordination with healthcare providers
- Assistance developing a care plan
- Connections to community-based services
- Respite care, including in-home respite
GUIDE generally requires Original Medicare (Parts A and B) and a dementia diagnosis. People enrolled in Medicare Advantage or receiving hospice care are not eligible.
Where Does In-Home Care Fit In?
Families supporting someone with dementia may also need assistance with everyday activities at home.
Non-medical home care can provide support with:
Companionship
Meal preparation
Personal care
Mobility assistance
Household tasks
Transportation
Routine and structure
Respite for family caregivers
The Medicare GUIDE Model provides dementia care coordination, caregiver education and support, and limited respite services for eligible participants. In-home respite care may be provided by home care agencies that partner with a GUIDE participant. Coverage and respite allowances are set by the participating GUIDE organization. Additional care beyond the respite benefit may be arranged through private pay.
