Recently, our own Dave Tasto was interviewed by Sydney Garsee of AxisCare regarding home care, Thriving at Home, and our efforts to track and reduce hospital readmissions here at Assisting Hands of Boston Northwest.
Their article brought out how we’ve used AxisCare’s Hospitalization Tracking software along with our own Thriving at Home approach to reduce our hospital readmission rate to well below the national average. This combined approach ultimately means delivering higher quality care for clients or their loved ones in their own homes, as well as reducing unnecessary, costly, and stressful trips to the hospital.
