Preventing hospitalizations, and keeping individuals out of the hospital is critically important. Particularly during the pandemic, and also now – no one wants to spend time in the hospital. A key metric that measures our performance is our Hospital Readmission Rate – which we are quite proud of, and have been recognized for having one of the lowest readmission rates in the country for non-medical home care – in the low single digits.
On average, nearly one in 5 Medicare patients goes back to the hospital within just 30 days after a previous discharge. Meaning that our likelihood of going back to the hospital is significantly increased after having just come out of the hospital. We probably all know friends or relatives who have been in the hospital recently…extra support and attention are needed right after a discharge to ensure a return trip can be avoided.
