Care Coordination

Initial Care Assessment

Our Owner or Care Coordinator visits each client at their home to assess their condition, discuss their options, and develop a Plan of Care.

Helping with Benefits

We assist our clients in identifying any programs, community resources, or benefits that may be available to them, such as, Private Health Insurance, Long Term Care Insurance, VA pensions, and others. We will work with your long-term care insurance provider to accept Assignment of Benefits and, if permitted by the carrier, will invoice providers directly. We relieve you of the burden of paperwork and make it as simple as possible for you to obtain any long-term care benefits.

Your initial Care Plan and how we keep it up to date

In order to develop an effective Plan of Care, we incorporate all orders and recommendations from your physicians, case managers, healthcare providers, and our own initial assessment. This plan will outline all of the services that are required, as well as the proposed frequency, cost of each service, potential equipment and supplies required, and potential payment sources. This process may begin before the client is discharged from the hospital, rehabilitation facility, or other healthcare facility. We will collaborate with the attending physician, discharge planner, family, and client to develop the best Plan of Care and agree on specific goals to accelerate the healing process.

 From Initial Assessment to Ongoing Care Management:

Every client is given a personalized Plan of Care. It is our way of ensuring that you receive the care you require and that everyone is prepared to provide it.

Step 1: The First Call and Meeting: We begin considering their loved one’s specific needs the moment they call our office. On the first call, we ask questions to determine your needs. Then we schedule an in-home visit with the family, including the family member who may benefit from our services in the future. This visit allows us to gather additional information, answer questions, and inform you about how Assisting Hands can help your loved one.

Step 2: In-Home Assessment: Our Care Coordinator or Owner visit each client and/or family member at their home to assess their condition, discuss options, and develop a Plan of Care.

Step 3: Plan of Care: In order to develop an effective Plan of Care, we incorporate all orders and recommendations from your physicians, case managers, healthcare providers, and our own initial assessment. This plan will outline all of the services that are required, as well as the proposed frequency, cost of each service, potential equipment and supplies required, and potential payment sources.

Step 4: Caregiver Assignment and Matching: When a new caregiver is assigned to a client, he or she is given a copy of the plan of care and meets with a nurse or care manager to discuss it. We are better able to provide your client with the best caregiver possible by considering the entire situation. Of course, if there are any concerns about a specific caregiver, we can quickly find and provide a replacement.

Step 5: Ongoing Management and Check-Ins: Care plans are constantly updated as a result of supervisory visits, caregiver experience on the case, and interactions with the client, healthcare professionals, or family members. Every update is distributed to all caregivers assigned to a client, and case managers follow up with individual aides to ensure that the updates are followed.

Management of Care:

Assisting Hands can provide Non-Medical Home Care services in the privacy and comfort of your own home. We will also assist in the coordination and referral of other services that may be required, such as:

Wolfgang Willems, the owner, and his team at Assisting Hands serving Frisco and Plano are top notch! We have been using them since July 2022 to help care for our mother who experienced life-changing illness in June. Coping with hospitals, insurance companies and rehab facilities has been so emotionally and physically draining on our family and we felt we were caught in a downward, never-ending spiral. Then someone told us about Assisting Hands. We did some research on them and other similar caregivers and finally decided on Assisting Hands. I cannot stress enough how wonderful they have been and how much they have helped our family. They have allowed us to share the burden of care and given us time to focus on the more important aspects of multiple therapies and recovery.

Assisting Hands Frisco is doing a phenomenal job by caring for my mother. It all started with both an easy but diligent intake done by Henny, their care manager. Already the next day after the intake, we had the first caregiver coming to my mother's home. Henny calls my mother and me every week just to keep everyone in the loop. The caregivers are on time and care for my mother as if she were a family member.
